This session will provide an overview of the AADE Health Care Outcomes Continuum as the foundation for outcomes unique to the role of the diabetes educator. The use of the AADE Health Care Outcomes provides a method of demonstrating the value of diabetes education in ensuring improved outcomes for the person with diabetes; improved learning skills and knowledge through to improved health status. It also serves as a valuable tool to educate other health care professionals about the role of CDEs. Also discussed will be the ADA/EASD Clinical Recommendations and the emphasis on individualizing treatment.
